Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, days

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in the comfort of their homes
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover from joint replacements and manage chronic conditions like CHF and COPD
Complete approximately 5 meaningful visits each day across the beautiful Tehama County area
Document care using the HomeCare HomeBase EMR system with your provided tablet
Collaborate with the intake RN and manager to coordinate excellent patient outcomes

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Valid driver's license and reliable transportation for home visits
Strong communication and time management skills

Physical Therapy assessment and treatment planning
Home Health documentation and compliance
Orthopedic rehabilitation techniques
Geriatric care with multiple comorbidities
Patient education and family communication
HCHB EMR system navigation
