Roles & Responsibilities

Deliver personalized physical therapy in patients' homes to help them regain strength and independence
Create customized treatment plans that meet each patient's unique goals and lifestyle
Educate patients and families on exercises and techniques to support recovery between visits
Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless care
Document progress and adjust care plans to keep patients on the path to wellness

Qualifications

Current California Physical Therapist license
BLS certification
Prior home health experience is a plus but not required
Strong communication skills and a passion for patient-centered care

Knowledge & Skills

Home health physical therapy best practices
Patient assessment and individualized care planning
Effective communication with patients, families, and care teams
Documentation and compliance standards
