Roles & Responsibilities

• Assess residents through thorough evaluations to identify physical therapy needs
• Develop and implement individualized treatment plans that improve mobility and independence
• Provide skilled therapy to enhance strength, balance, and functional outcomes
• Document evaluations, daily treatment notes, progress notes, and discharge summaries accurately and on time
• Collaborate with interdisciplinary team members to coordinate patient care
• Educate patients and caregivers on therapeutic exercises and safety techniques
• Ensure compliance with Medicare, Medi-Cal, and facility regulatory standards

Qualifications

• Active California Physical Therapist (PT) license
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Previous skilled nursing facility or post-acute experience preferred but not required
• Strong documentation and time management skills
• Ability to work independently in a dynamic environment and collaborate with a team
