Roles & Responsibilities

• Conduct thorough occupational therapy evaluations and create individualized care plans that promote independence
• Provide skilled OT interventions to improve activities of daily living, mobility, and patient safety
• Collaborate with interdisciplinary healthcare teams to coordinate effective, patient-centered care
• Maintain accurate and timely documentation in compliance with state and facility guidelines
• Educate patients and families on therapy goals, progress, and home strategies to support long-term gains
• Use clinical judgment to adapt treatment plans and respond to changing patient needs

Qualifications

• Active California Occupational Therapist license required
• SNF or geriatric experience preferred; new graduates are encouraged to apply
• Strong clinical skills with a focus on patient-centered rehabilitation
• Excellent communication skills for working with patients, families, and the care team
• Organizational skills and reliable documentation practices to meet regulatory standards
• Ability to work collaboratively in an interdisciplinary skilled nursing facility setting

Knowledge & Skills

• Occupational therapy assessment and treatment planning
• Knowledge of ADLs, mobility training, and safety interventions
• Patient care and clinical documentation best practices
• Interdisciplinary teamwork and effective communication
• Time management and organizational skills for therapy scheduling and charting
• Ability to educate patients and families in approachable, practical ways
