Roles & Responsibilities

• Evaluate patients' functional status and interpret physician plans of care
• Develop and implement individualized treatment plans to restore meaningful daily activities
• Conduct assessments and reassessments using objective tests and measurements
• Communicate observations and treatment suggestions as an interdisciplinary team member
• Provide patient and caregiver education to promote safety and independence at home
• Complete accurate, concise clinical documentation and coordinate care activities
• Respond and adapt to changes in patient condition and emergency situations
• Travel to patients' homes safely and provide compassionate, in-home care

Qualifications

• Licensed Occupational Therapist in the State of Oregon (required)
• Graduate of an accredited occupational therapy curriculum (required)
• Eligible for the National Registration Examination of the American Occupational Therapy Association (required)
• Current BLS certification (required)
• Valid driver’s license and proof of auto insurance (required for in-home care)
• Home health and hospice experience preferred
• Registration with the American Occupational Therapy Association (AOTA) preferred
• CRASE training required if the travel assignment extends beyond six months
• Evidence of continuing education relevant to your practice and patient population

Education

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Eligible for the National Registration Examination of the American Occupational Therapy Association

Knowledge & Skills

• Patient evaluation and assessment/reassessment
• Treatment planning for self-care, leisure, and work activities
• Clinical documentation and care coordination
• Interdisciplinary communication and teamwork
• Patient and caregiver education
• Geriatric care, rehabilitation procedures, and physical disability knowledge
• Familiarity with mental health concepts as they relate to occupational therapy
• Ability to work independently and adapt in emergency or changing situations
• Experience delivering in-home patient care in Home Health and Hospice settings
