Roles & Responsibilities

• Conduct thorough occupational therapy evaluations to establish individualized goals
• Provide daily treatments focused on functional outcomes and safe discharge home
• Manage and coordinate discharges to ensure smooth transitions back to the community
• Collaborate closely with the rehab team to design and implement effective care plans
• Maintain documentation and productivity standards (facility notes a 92% productivity requirement)
• Adhere to facility policies and procedures and represent the therapy department professionally (black scrubs dress code)

Qualifications

• Occupational Therapy experience (job lists OT as primary skill)
• 1-3 years of experience preferred (job indicates 1-3 years)
• Prior SNF experience strongly preferred (explicitly noted)
• Ability to work on-site and be present in the building to receive guaranteed hours
• Willingness to meet facility productivity expectations and follow CRG policies (92% productivity requirement)
• Available to start around the requested start date or within 2-3 weeks as noted

Knowledge & Skills

• Skilled in OT evaluations and writing clear, goal-directed treatment plans
• Experienced delivering daily treatments in a skilled nursing facility setting
• Knowledge of discharge planning and rehab-to-home focused interventions
• Comfort working in a SNF environment with a high rehab-to-home census
• Strong documentation, time-management, and teamwork skills
