Roles & Responsibilities

• Provide compassionate, skilled nursing care to home health patients as a Licensed Practical Nurse
• Administer medications, perform wound care, and follow established care plans safely and accurately
• Perform BLS-level patient assessments and intervene appropriately for changes in condition
• Document patient status, visits, and clinical findings clearly and promptly in the chart
• Educate patients and caregivers on disease management, medications, and home safety
• Coordinate with RNs, therapists, and the interdisciplinary team to ensure seamless care
• Complete Trinity specialty competency testing and participate in required training
• Adhere to infection control, immunization, and fit test requirements to keep patients and staff safe

Qualifications

• Active LPN license in Michigan (or eligible to obtain prior to start)
• Current BLS certification
• Successful License/Certificate Verification required
• Willingness to complete Trinity Specialty Specific Competency Testing
• Ability to pass required health screenings: Fit Test, 2-step PPD or IGRA, Trinity 10-panel drug screen, respirator evaluation
• Up-to-date immunizations/documentation: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine
• Reliable transportation and ability to travel within assigned home health territory

Education

• Licensed Practical Nurse (LPN) credential from an accredited program; current Michigan LPN license preferred

Knowledge & Skills

• Strong clinical skills in home health nursing and patient-centered care
• Comfortable with medication administration and wound care procedures
• Excellent communication and caregiver teaching abilities
• Good time management and ability to prioritize caseloads
• Familiarity with electronic documentation and care coordination
• Ability to work independently in patients' homes while following agency protocols
