Roles & Responsibilities

Provide compassionate direct patient care consistent with LPN scope of practice
Administer medications and treatments accurately and document appropriately
Monitor patient status and report changes to the registered nurse or provider
Perform wound care, basic assessments, and routine procedures as required
Maintain clear, timely charting in the patient record and communicate handoffs
Collaborate with the interdisciplinary team to support care plans and patient goals
Support patient education and family communication with empathy and clarity

Qualifications

Current, active LPN license in Pennsylvania (required)
CPR/BLS certification (or willingness to obtain upon hire)
Comfortable administering medications and performing basic nursing procedures
Strong communication skills and ability to work well on a collaborative care team
Professional attitude, reliability, and a patient-first mindset

Education

Graduate of an accredited Practical Nursing program with LPN certification

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and infection control principles
Vital signs monitoring and recognition of patient status changes
Familiarity with electronic health records and accurate charting
Time management, prioritization, and teamwork in a clinical setting
