Roles & Responsibilities
- Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
- Administer medications and treatments accurately and document per facility policy
- Perform timely patient assessments and report changes in condition to the RN or provider
- Maintain clear, thorough documentation in the patient chart and electronic health record
- Collaborate with the interdisciplinary care team to develop and follow patient care plans
- Support admissions, discharges, and safe patient transfers with attention to detail
- Assist in patient and family education to promote recovery, safety, and wellness
Qualifications
- Current, active LPN license in Pennsylvania (required)
- Current BLS/CPR certification or willingness to obtain upon hire
- Comfortable working in long-term care or skilled nursing environments
- Clear background check and drug screen as required by the facility
- Strong communication skills and a team-oriented mindset
- Previous clinical experience preferred; new grads considered with relevant training
Education
- Graduate of an accredited Practical Nursing (LPN/LVN) program
Knowledge & Skills
- Fundamental nursing assessments and vital sign monitoring
- Medication administration and basic wound care
- Familiarity with charting and documentation best practices
- Infection control and patient safety principles
- Ability to collaborate with nurses, aides, therapists, and providers
- Time management and prioritization in a busy clinical setting
