Roles & Responsibilities

• Provide compassionate direct patient care within scope of LPN practice — medication administration, wound care, and routine clinical assessments
• Collaborate with RNs, physicians, and interdisciplinary teams to implement care plans and ensure continuity
• Document care accurately and promptly in the patient record to support safe handoffs
• Monitor patients for changes in condition and escalate concerns in a timely manner
• Support patient education and family communication with clear, kind explanations
• Assist with admissions, discharges, and safe patient transfers while maintaining dignity and respect

Qualifications

• Active LPN license in Pennsylvania (PA) or eligible to obtain one — state licensure required
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Demonstrated clinical competence as a Licensed Practical Nurse; previous long-term care or facility experience is a plus
• Up-to-date immunizations and ability to pass background checks and required health screenings
• Strong communication skills, teamwork mindset, and commitment to patient-centered care

Education

• LPN diploma or certificate from an accredited program
• High school diploma or GED

Knowledge & Skills

• Medication administration and accurate charting
• Basic wound care and vital signs assessment
• Familiarity with infection control and safety protocols
• Time management and ability to prioritize multiple patient needs
• Comfort working in a fast-paced clinical setting and collaborating with team members
