Roles & Responsibilities

Provide compassionate direct patient care following care plans and facility policies
Administer medications and treatments as directed within LPN scope
Monitor patient condition and promptly report changes to the RN or provider
Document care, observations, and patient responses accurately and timely
Collaborate with the care team to ensure safe, high-quality patient outcomes
Assist residents with activities of daily living and promote a comfortable environment

Qualifications

Active LPN license in Pennsylvania (or applicable compact state credentials)
Graduation from an accredited practical nursing program
Current CPR/BLS certification preferred
Ability to pass background checks and meet facility onboarding requirements
Strong communication skills, compassion, and ability to work well on a team

Education

High school diploma or GED; completion of an accredited practical nursing program preferred

Knowledge & Skills

Medication administration and basic nursing procedures
Patient assessment and vital signs monitoring
Clinical documentation and charting
Infection control and safety practices
Effective interpersonal communication and resident-centered care
