Roles & Responsibilities
- Provide compassionate direct patient care, monitoring patients and reporting changes promptly
- Administer medications and document treatments accurately according to facility policy
- Assist with patient assessments, basic procedures, and care plan implementation
- Collaborate with RNs, physicians, and interdisciplinary team members to support quality outcomes
- Maintain accurate and timely documentation in the electronic health record
- Support patient and family education with clear, empathetic communication
Qualifications
- Active LPN license in Pennsylvania or eligibility to obtain licensure
- Current CPR/BLS certification (or willingness to obtain)
- Comfortable working in a fast-paced, patient-focused environment
- Strong communication skills and the ability to work collaboratively with clinical teams
- Reliable, professional, and dedicated to delivering compassionate care
Education
- Practical Nursing diploma or equivalent training required
Knowledge & Skills
- Medication administration and basic nursing procedures
- Patient assessment and vital sign monitoring
- Electronic health record (EHR) documentation
- Infection control practices and patient safety principles
- Time management and the ability to prioritize patient needs
