Roles & Responsibilities

• Provide compassionate direct care to PACE participants, including medication administration, basic wound care, and assistance with activities of daily living
• Perform accurate assessments and document changes in participant condition in the medical record
• Collaborate with an interdisciplinary team to support individualized care plans and coordinate participant needs
• Educate participants and families on care plans, safety measures, and health maintenance
• Follow infection control protocols, complete required screenings and maintain up-to-date immunizations
• Complete Trinity Skills Checklist and participate in competency evaluations to ensure high-quality care

Qualifications

• Active, verifiable LPN license (License/Certificate Verification required)
• Current Basic Life Support (BLS) certification
• Completion of Trinity Skills Checklist and successful credentialing
• Required health screenings and immunizations: Fit Test, 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 and Influenza vaccines, Trinity 10 Panel Drug Screen, and a Physical/Health Statement
• Comfortable working in a per diem/pool model with flexible availability
• Excellent communication, organization, and teamwork skills

Education

• Completion of an accredited Practical Nursing program
• Diploma or certificate qualifying for LPN licensure

Knowledge & Skills

• Patient assessment and monitoring (vital signs, basic clinical observations)
• Medication administration and documentation
• Basic wound care and ADL support
• Infection prevention and control practices
• Electronic health record documentation and care coordination
• Understanding of geriatric care needs and chronic disease management
• Strong interpersonal skills for working with participants, families, and the care team
