Roles & Responsibilities

Provide compassionate direct patient care and monitor residents’ clinical status
Administer medications accurately and document treatments in the chart
Perform thorough assessments and communicate changes to the care team
Collaborate with nurses, therapists, and providers to implement individualized care plans
Maintain infection control practices and promote a safe healing environment
Support residents’ daily living needs with dignity and respect

Qualifications

Active Licensed Practical Nurse credential (LPN)
Previous LPN experience in long-term care or skilled nursing preferred
Current BLS/CPR certification or willingness to obtain prior to start
Reliable transportation — local assignment requires you have your own vehicle
Ability to obtain any facility-required screenings and approvals before start

Education

Completed accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

Strong skills in medication administration and basic clinical procedures
Comfortable with resident assessment, documentation, and care planning
Good communication and teamwork to coordinate with interdisciplinary staff
Knowledge of infection control principles and safe patient handling
Ability to learn facility-specific electronic charting systems
