Roles & Responsibilities

• Provide compassionate, patient-centered direct care to residents and support their dignity
• Administer medications and treatments safely and accurately according to physician orders
• Monitor, record, and report vital signs and changes in condition in a timely manner
• Collaborate with nurses, physicians, and care partners to implement individualized care plans
• Assist residents with activities of daily living while encouraging independence
• Maintain accurate and timely documentation in the medical record and electronic systems
• Follow infection control practices and facility policies to keep residents and staff safe

Qualifications

• Current Licensed Practical Nurse (LPN) license in New Hampshire (required)
• Current BLS/CPR certification or ability to obtain upon hire
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Ability to pass background checks and meet facility onboarding requirements
• Strong communication, teamwork, and time-management skills

Education

• Graduate of an accredited LPN/Licensed Practical Nurse program or vocational school

Knowledge & Skills

• Medication administration and safe documentation practices
• Patient assessment, clinical observation, and escalation of concerns
• Infection control protocols and safe patient handling
• Comfortable using electronic health records and charting accurately
• Prioritization and multitasking in a busy clinical environment
• Compassionate bedside manner and clear communication with residents and families
