Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, ensuring safety and comfort
• Administer medications and treatments accurately and on schedule
• Monitor and document resident conditions, reporting changes promptly to the RN and care team
• Support individualized care plans and collaborate with interdisciplinary staff to meet resident needs
• Assist with ADLs (activities of daily living) while promoting dignity and independence
• Educate residents and families about care plans and daily routines in a clear, respectful way
• Maintain accurate, timely electronic and paper documentation in accordance with facility policy

Qualifications

• Active LPN license in Pennsylvania (required)
• Completion of an accredited practical/vocational nursing program
• Current BLS/CPR certification (or ability to obtain upon hire)
• Strong communication skills and a teamwork-focused attitude
• Prior experience in long-term care or skilled nursing preferred but new graduates may be considered
• Ability to pass required background checks and health screenings

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

• Medication administration and basic clinical assessment skills
• Wound care basics and infection control practices
• Accurate charting and familiarity with electronic medical records (EMR)
• Patient safety, fall prevention, and care planning
• Time management and the ability to prioritize in a fast-paced setting
• Compassionate resident-centered care and strong interpersonal skills
