Roles & Responsibilities

Provide compassionate, resident-centered nursing care in a long-term care setting
Administer medications and treatments safely and in accordance with facility policy
Monitor resident condition, document observations, and communicate changes to the care team
Collaborate with RNs, CNAs, and interdisciplinary staff to deliver coordinated care
Support residents with activities of daily living while preserving dignity and independence
Participate in care planning, charting, and regulatory documentation to maintain high-quality standards

Qualifications

Active LPN license in Pennsylvania (required)
LPN diploma or certificate from an accredited program
Current BLS/CPR certification (or willingness to obtain upon hire)
Clear background check and any required facility screenings
Strong communication skills and a resident-first mindset

Education

High school diploma or equivalent; completion of an accredited LPN program

Knowledge & Skills

Medication administration and documentation best practices
Basic nursing assessments and vital sign monitoring
Knowledge of long-term care workflows and resident-centered care
Basic infection control and safety procedures
Ability to work collaboratively with a multidisciplinary team
