Roles & Responsibilities

• Provide compassionate, patient-centered nursing care for assigned residents in accordance with facility policies
• Administer medications and treatments accurately and document thoroughly
• Monitor patient condition, recognize changes, and escalate concerns to the care team promptly
• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Perform thorough shift handoffs and maintain clear, timely clinical documentation
• Support a positive patient experience by educating patients and families and advocating for their needs

Qualifications

• Active LPN license valid in New Hampshire (LPN) — required
• Clinical experience in long-term care, medical-surgical, or similar acute care settings is a plus
• Current BLS (Basic Life Support) certification or willingness to obtain prior to start
• Excellent communication skills and a teamwork mindset
• Ability to work scheduled 12+ hour day shifts, including every-other-weekend coverage

Education

• Completion of an accredited practical nursing program (LPN)
• For RNs: ADN or BSN if applying as Registered Nurse

Knowledge & Skills

• Strong medication administration and documentation skills
• Assessment skills to recognize and respond to changes in patient condition
• Familiarity with electronic medical records and accurate charting practices
• Comfort working in a fast-paced environment and prioritizing patient safety
• Compassionate bedside manner and the ability to educate patients and families
