Roles & Responsibilities

• Provide compassionate, patient-focused nursing care as an LPN at the Sandia Ridge Center
• Administer medications and treatments accurately and document per facility policy
• Monitor patient condition and report changes to the care team promptly
• Assist with daily living activities and support patient comfort and dignity
• Collaborate with RNs, physicians, and interdisciplinary staff to deliver coordinated care
• Maintain clear, timely documentation in the patient record and follow infection control protocols
• Support admissions, discharges, and effective patient handoffs during shift changes

Qualifications

• Active Licensed Practical Nurse (LPN) license; state-appropriate licensure required (New Mexico license if practicing in NM)
• Completion of an accredited LPN/vocational nursing program
• Basic Life Support (BLS) certification preferred or ability to obtain upon hire
• Clinical experience in long-term care, skilled nursing, rehab, or similar settings preferred
• Strong communication skills and ability to work well within a multidisciplinary team

Education

• Graduate of an accredited LPN/vocational nursing program; high school diploma or GED required

Knowledge & Skills

• Medication administration and basic clinical assessments
• Wound care basics and assistance with ADLs
• Patient safety, fall prevention, and infection control practices
• Clear documentation and charting skills
• Team collaboration, adaptability, and time management
• Comfort using electronic medical records and following facility policies
