Roles & Responsibilities

• Provide compassionate, person-centered care to residents, promoting dignity and independence
• Assist with activities of daily living including bathing, dressing, toileting, and mobility support
• Perform vital signs and routine observations, reporting any changes in condition to the nursing team
• Document care accurately and promptly in resident charts to ensure continuity and safety
• Support safe transfers and use proper body mechanics to protect residents and yourself
• Collaborate with nurses, therapists, and care staff to deliver coordinated, high-quality care
• Participate in care planning and follow care plans to meet individual resident needs
• Maintain infection control standards and a clean, safe environment for residents

Qualifications

• Current LNA (Licensed Nursing Assistant) certification; valid in New Hampshire (VT certification a plus)
• CPR/BLS certification (current) or willingness to obtain upon hire
• Minimum experience in long-term care or similar setting preferred but new grads welcome
• Up-to-date immunizations and ability to pass background and health screenings
• Strong communication skills and a team-first attitude
• Reliable, punctual, and able to work the assigned rotating schedule

Education

• Completion of a state-approved nursing assistant program
• High school diploma or GED preferred

Knowledge & Skills

• Hands-on resident care and ADL assistance
• Basic clinical skills: vital signs, documentation, observation
• Infection prevention and safety best practices
• Comfortable with electronic charting or paper chart systems
• Effective verbal communication with residents and care teams
• Time management and ability to prioritize resident needs during shift
