Roles & Responsibilities

• Provide clinical pharmacy services in an outpatient oncology infusion setting to optimize patient outcomes
• Perform clinical dosing, including pharmacokinetic dosing and therapeutic drug monitoring, to ensure safe and effective therapy
• Review and verify chemotherapy and supportive care medication orders for accuracy and safety
• Prepare and oversee sterile compounding of IV medications, maintaining strict aseptic technique and safety standards
• Administer and monitor IV piggyback and IV push medications, watching for efficacy and adverse effects
• Collaborate with oncologists, nurses, and interdisciplinary teams to coordinate patient-centered care plans
• Use Epic EMR for order verification, documentation, and medication management to maintain accurate patient records
• Ensure compliance with oncology safety standards and protocols to protect patients and staff
• Educate patients on oncology therapies and adverse effect management to empower informed care decisions

Qualifications

• Active Pharmacist license in California required or ability to obtain prior to start
• Demonstrated recent experience in oncology clinical infusion practice
• Experience with sterile compounding and IV medication preparation
• Proficiency with Epic EMR for order verification and documentation
• Strong collaboration and communication skills for interdisciplinary teamwork
• Preferred: training or familiarity with pharmacokinetic dosing and therapeutic drug monitoring

Education

• Doctor of Pharmacy (PharmD) preferred; equivalent pharmacy degree considered
