Roles & Responsibilities

• Provide compassionate, hands-on care to participants as a trusted CNA
• Assist with activities of daily living (bathing, dressing, toileting, mobility)
• Take and record vital signs, observing changes and reporting concerns promptly
• Support meal assistance and proper feeding when required
• Follow infection control practices and maintain a safe, clean environment
• Complete documentation accurately and on time to support continuity of care
• Participate in required Trinity Skills Checklist and maintain competency
• Collaborate with nurses and the interdisciplinary team to deliver person-centered care

Qualifications

• Current Certified Nursing Assistant (CNA) certification
• BLS (Basic Life Support) certification
• Successful completion of Trinity Skills Checklist and license/certificate verification
• Up-to-date immunizations and health screening (2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, Influenza) as required
• Clear Trinity 10 panel drug screen and physical/health statement
• Comfort working in a per diem PD Pool and flexibility to pick varied shifts

Education

• Certified Nursing Assistant (CNA) certification

Knowledge & Skills

• Fundamental patient care skills and safe transfer/ambulation techniques
• Accurate vital sign measurement and documentation
• Strong communication and teamwork with clinical staff
• Infection prevention and control practices
• Time management and the ability to prioritize patient needs
• Attention to detail for charting, medication observation, and care plans
