Roles & Responsibilities

• Provide compassionate hands-on care to PACE participants, assisting with activities of daily living
• Support safe, person-centered care by following care plans and communicating changes to the team
• Perform timely documentation and charting to keep participant records accurate and up to date
• Assist with mobility, feeding, hygiene, and other direct care tasks while preserving dignity
• Participate in infection control practices and ensure immunization and screening compliance
• Collaborate with interdisciplinary team members to support quality outcomes for participants

Qualifications

• Certified Nursing Assistant (CNA) certification required
• Must meet immunization requirements: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine
• Ability to pass Trinity 10-panel drug screen and provide a Physical/Health Statement
• Comfort working in a per diem PD pool environment and adapting to variable schedules
• Strong communication skills and a team-oriented attitude

Education

• Certified Nursing Assistant (CNA) certification

Knowledge & Skills

• Practical experience with hands-on patient care and ADLs
• Basic infection control and immunization awareness
• Reliable documentation and time-management skills
• Ability to follow care plans and escalate changes appropriately
• Compassionate bedside manner and respect for participant dignity
