Roles & Responsibilities

• Provide compassionate direct care to PACE participants, supporting dignity and independence
• Assist with activities of daily living (bathing, dressing, grooming, toileting, feeding)
• Monitor participants for changes in condition and report concerns to nursing staff promptly
• Perform basic clinical tasks per scope of practice, including vital signs and documentation
• Complete required Trinity Skills Checklist and participate in competency verification
• Follow infection control protocols and maintain a safe environment for participants
• Communicate clearly and kindly with participants, families, and interdisciplinary team members

Qualifications

• Current Certified Nursing Assistant (CNA) certification in Maryland
• BLS (Basic Life Support) required
• NRP (Neonatal Resuscitation Program) required
• TNCC (Trauma Nursing Core Course) required
• Successful completion of Trinity Skills Checklist and license/certificate verification
• Meet health requirements: Fit Test; 2-step PPD or IGRA; MMR, Hepatitis B, Varicella, Tdap; COVID-19 and Influenza vaccination; Trinity 10-panel drug screen
• Reliable, compassionate, and team-oriented with good communication skills

Education

• High school diploma or equivalent; active CNA certification required

Knowledge & Skills

• Strong fundamentals of patient care and activities of daily living
• Infection prevention and standard precautions
• Basic vital sign measurement and patient observation
• Comfortable with documentation and following care plans
• Able to respond calmly in emergencies and follow BLS/NRP/TNCC guidance
• Team collaboration and clear communication with clinical staff
