Roles & Responsibilities

• Provide compassionate assistance with activities of daily living, promoting dignity and independence
• Monitor and record vital signs and patient responses accurately
• Support safe mobility, transfers and falls prevention under established protocols
• Assist the interdisciplinary team with care plans and timely documentation
• Maintain a clean, safe environment and observe infection control practices
• Build rapport with participants and family members through clear, respectful communication
• Participate in trainings, fit testing, and required health screenings to ensure readiness for shifts

Qualifications

• Active CNA certification or equivalent (state requirements will be verified)
• BLS certification (required)
• License/Certificate Verification completed prior to assignment
• Ability to meet Trinity Health personal health requirements (vaccines, TB screening, drug screen, fit test, etc.)
• Comfort working in a person-centered care model with older adults
• Reliable, professional, and able to follow care plans and facility procedures

Education

• High school diploma or GED preferred
• State CNA certification or completion of an approved CNA training program

Knowledge & Skills

• Strong basic clinical skills: ADLs, vital signs, transfers and infection control
• Clear, compassionate communication with participants, families, and the care team
• Accurate documentation and attention to detail
• Ability to follow care plans and escalate concerns appropriately
• Teamwork and flexibility in a dynamic care environment
• Respect for privacy, dignity and cultural differences
