Roles & Responsibilities

• Provide compassionate, hands-on personal care to residents including bathing, grooming, and mobility assistance
• Monitor and record vital signs and resident conditions, escalating concerns to nursing staff promptly
• Support activities of daily living while promoting independence and dignity
• Complete required documentation accurately and timely to support continuity of care
• Assist with safe transfers and positioning to prevent injury and maintain comfort
• Participate in mandatory competency testing and skills checklists to ensure high-quality care
• Work collaboratively with interdisciplinary team members to deliver resident-centered care

Qualifications

• Valid CNA license/certification as required by the facility
• BLS certification (required)
• Successful completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Ability to pass license and certificate verification
• Current immunizations and health requirements: 2 Step PPD or IGRA (Quantiferon), MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, and annual influenza vaccine
• Successful Trinity 10 panel drug screen and physical/health statement

Knowledge & Skills

• Strong resident-focused bedside care skills
• Knowledge of infection control practices and standard precautions
• Comfortable with documentation in clinical charts and shift reports
• Good communication and teamwork skills with residents, families, and staff
• Ability to follow care plans and report changes in condition
• Competency in safe patient handling and mobility support
