Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living
• Assist residents with hygiene, mobility, feeding, and toileting needs
• Accurately document resident care and report changes in condition to nursing staff
• Follow infection control protocols and maintain a safe, clean environment
• Participate in Trinity specialty competency testing and complete required skills checklists
• Support interdisciplinary team communication to promote resident-centered care
• Respect resident dignity and privacy while promoting independence

Qualifications

• Current CNA certification (state-appropriate) required
• BLS certification required
• Ability to pass Trinity specialty competency testing and Trinity skills checklist
• Completion of required health screenings and vaccinations (PPD/IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
• Successful completion of background checks, 10-panel drug screen, and license/certificate verification
• Previous long-term care or home health experience preferred but not required

Education

• High school diploma or equivalent
• Completion of an approved CNA training program

Knowledge & Skills

• Strong patient care and bedside skills for assisting with ADLs
• Solid infection control and safety practices
• Clear verbal and written communication for documentation and team updates
• Basic mobility and transfer techniques
• Comfortable with competency-based testing and ongoing skills validation
• Empathy, patience, and the ability to work collaboratively
