Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living (ADLs)
• Monitor and document vital signs, reporting changes promptly to nursing staff
• Assist with safe transfers, mobility and use of assistive devices to support patient safety
• Support rehabilitation goals in collaboration with nurses and therapy teams at Canyon Trans Rhb Ctr
• Maintain infection control standards and keep patient areas clean and safe
• Accurately document care in patient records and communicate clearly with the care team
• Build trusting relationships with patients and families, preserving dignity and respect

Qualifications

• Active Certified Nursing Assistant (CNA) certification in good standing
• CPR/BLS certification preferred or willingness to obtain
• Experience in rehab or long-term care is a plus; new grads encouraged to apply
• Able to work scheduled 12-hour day shifts (Friday, Saturday, Sunday)
• Must meet facility-required background and immunization policies

Education

• High school diploma or equivalent and completion of an accredited CNA program

Knowledge & Skills

• Strong patient-centered care skills and assistance with ADLs
• Accurate vital signs measurement and clinical documentation
• Safe transfer and mobility techniques
• Infection prevention and proper PPE use
• Clear communication, teamwork and the ability to follow care plans
