Case Manager - Hospice

Location: Grand Junction, Colorado

Start Date: 07-13-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating on-call and weekend coverage

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide compassionate end-of-life care to patients and their families in private homes, nursing homes, group homes, or assisted living facilities
Lead interdisciplinary team meetings to coordinate and personalize care plans
Educate patients and caregivers on disease progression, symptom management, and care techniques
Conduct comprehensive assessments and apply critical thinking to manage symptoms effectively
Maintain accurate patient records and documentation to ensure continuity of care
Support patients and families with empathy, strength, and courage during challenging times

Qualifications

Must have at least 2 years of hospice experience
Active RN license in Colorado (must be currently active)
Excellent clinical assessment and critical thinking skills
Ability to work independently and as part of a collaborative team
Passion for hospice care and making a meaningful difference in patients' lives

Strong communication and interpersonal skills
