Roles & Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans based on their functional needs and goals
Provide evidence-based physical therapy interventions to improve mobility, strength, balance, endurance, and overall function
Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes
Educate patients, families, and caregivers on therapeutic exercises, mobility techniques, and injury prevention strategies
Collaborate with physicians, nursing staff, and interdisciplinary team members to ensure coordinated patient care
Maintain accurate and timely documentation in compliance with facility and regulatory requirements

Qualifications

Active Physical Therapist (PT) license in California
Doctor of Physical Therapy (DPT) degree from an accredited program
Current CPR/BLS certification
Experience in skilled nursing facility preferred but not required
Strong clinical, communication, and interpersonal skills

Education

Doctor of Physical Therapy (DPT) degree from an accredited program

Knowledge & Skills

Excellent communication and interpersonal skills
Ability to collaborate with interdisciplinary teams
Knowledge of evidence-based physical therapy practices
Proficient in patient evaluation and treatment planning
CPR/BLS certified
