Roles & Responsibilities

• Perform thorough SLP evaluations to identify patient needs and craft individualized plans of care
• Deliver engaging, evidence-based daily treatments that help residents regain function and confidence
• Manage timely and effective discharge planning to support safe transitions
• Maintain documentation and productivity standards (facility requires ~90% productivity) with accuracy and professionalism
• Collaborate closely with the interdisciplinary therapy team to coordinate care and drive outcomes
• Follow CRG policies and procedures to ensure consistent, high-quality service delivery
• Represent the facility by maintaining a professional appearance (black scrubs) and reliable on-site presence

Qualifications

• 1–3 years of clinical SLP experience (as listed in the job details)
• Prior SNF (skilled nursing facility) experience is strongly preferred
• Valid SLP credentials or licensure in the state of New Mexico, or the ability to obtain state licensure
• Comfortable following facility policies (CRG P&P) and meeting productivity expectations
• Strong clinical documentation skills and the ability to collaborate with a multidisciplinary team
• Dependable, professional presence on-site to receive guaranteed hours

Knowledge & Skills

• Clinical evaluations and evidence-based treatment planning for adult/geriatric populations
• Daily therapy delivery in a skilled nursing setting
• Discharge planning and transition coordination
• Productivity tracking and time management to meet facility targets
• Thorough clinical documentation and regulatory compliance
