Roles & Responsibilities

Conduct thorough clinical assessments, screenings, diagnoses, and treatment planning for patients from birth to elders
Deliver brief therapy and continuing care to strengthen families and support cultural wellness
Lead discharge planning and facilitate patient care conferences for smooth transitions to ALF or transitional placements
Provide expert care coordination, client advocacy, and skill-building across multidisciplinary teams
Offer crisis intervention, chemical dependency support, mental health referrals, and Power of Attorney assistance
Maintain precise, compliant documentation including progress notes, reports, and billing standards
Collaborate with community partners to ensure optimal patient outcomes and advance Tribal health missions

Qualifications

Masters Degree in Social Work, Psychology, Counseling or related discipline
Licensed as or eligible for LCSW, LPC, or LMFT
Valid Alaska Driver's License (obtainable within 6 months of hire)
Must be 21 years of age
Clinical Competency required within 3 months of hire

Education

Masters Degree in Social Work, Psychology, Counseling or related discipline from an accredited institution

Knowledge & Skills

Discharge planning and interviewing techniques
Crisis intervention and ethical/legal practices
Case management with tact and independent judgment
Clinical assessment, treatment planning, and documentation
Multi-disciplinary collaboration and patient advocacy
