Roles & Responsibilities

• Evaluate residents and create individualized speech-language therapy plans
• Provide evidence-based SLP interventions for swallowing, communication, and cognitive-communication disorders
• Document timely and accurate treatment notes and progress towards goals
• Collaborate with nursing and interdisciplinary team members to coordinate care
• Meet facility productivity expectations (90% productivity requirement)
• Follow Continuum Rehabilitation Group / CRG policies & procedures and facility protocols
• Participate in team meetings and support continuity of care within the SNF

Qualifications

• Current state SLP license (required to practice in New Mexico)
• ASHA CCC-SLP or working toward certification is a plus
• 1-3 years of clinical SLP experience (as listed in the job order)
• Experience or comfort working in a skilled nursing facility (SNF) setting
• Reliable transportation and ability to be present in the building to receive guaranteed hours
• Willingness to follow dress code (black scrubs) and facility policies

Knowledge & Skills

• Strong assessment and treatment planning skills for dysphagia and communication disorders
• Skilled in clinical documentation and productivity-focused workflows
• Ability to work collaboratively with rehab team members and nursing staff
• Good time management, organizational skills, and professional communication
• Familiarity with SNF regulatory/documentation expectations and therapy best practices
