Roles & Responsibilities

Deliver vital education and case management to patients with diabetes, prediabetes, and gestational diabetes
Maintain accurate diabetes registries to track and support patient care
Ensure patients receive essential annual exams, lab tests, immunizations, and education per IHS Standards
Coordinate primary and secondary prevention goals alongside the SEARHC Diabetes team
Facilitate training on IHS Standards of Care for providers, dietitians, nurses, and staff
Utilize audit data and tools to deliver top-quality care and reduce chronic costs
Answer inquiries and relay messages with friendly, cooperative service
Model SEARHC Seven Standards of Excellence in every interaction

Qualifications

Valid current RN license in Alaska
Certified Diabetes Educator or eligible to apply within 2 years
Clinical Competency Assessment - SEARHC within 3 months of hire and every three years
3 years general nursing or dietetic experience preferred
1 year in diabetic care and management preferred

Education

RN license required

Knowledge & Skills

Diabetes case management practices and procedures
Health promotion and patient education
Customs of Alaska Native groups
Performing diabetic care procedures with professional knowledge
Tact and sensitivity in stressful situations
Oral and written communication
Computer use
Teaching and organization
