Roles & Responsibilities

• Coordinate and manage patient care as the unit Care Coordinator — oversee caseloads of roughly 15–25 patients daily
• Perform case management tasks including continuity of care, discharge planning, and interdisciplinary communication
• Chart and document patient care accurately using Cerner
• Float confidently across acute care units (excluding surgery) to meet patient and staffing needs
• Collaborate with physicians, social work, and other teams to ensure timely transitions and quality outcomes
• Provide clear, compassionate patient and family education and support during care transitions
• Maintain required certifications and participate in the one-day RNCC orientation to ramp up quickly

Qualifications

• Active California Registered Nurse (RN) license required
• Recent experience as a Care Coordinator at a California Dignity Health/CommonSpirit hospital is REQUIRED
• Minimum ~2 years of nursing experience (acute care/case management experience preferred)
• BLS certification required
• Must provide 2 supervisory references at submittal (one within 12 months and one within 3 years)
• Profiles go directly to the manager for consideration; candidates employed by CommonSpirit/Dignity within the past year are not eligible unless PRN through an outside firm

Education

• Active California RN license (ADN or BSN acceptable) — BSN not required
• 12-hour unit orientation with RNCC (provided)

Knowledge & Skills

• Strong case management and care coordination skills
• Proficiency with Cerner charting system
• Comfort with acute care patient populations (cardiac, surgical, oncology, critical care)
• Adaptability and clinical judgment for floating to multiple units
• Excellent communication, organization, and interdisciplinary collaboration skills
• Ability to manage moderate caseloads while prioritizing safe discharges
