Roles & Responsibilities

Deliver compassionate physical therapy care to patients across greater Sacramento county in their homes
Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
Help patients regain strength, mobility, and independence with personalized treatment plans
Collaborate with interdisciplinary teams to ensure the best outcomes for every individual
Document visits thoroughly and maintain clear communication with supervisors

Qualifications

Active Physical Therapy Assistant license in California
Prior home health experience preferred
New grads considered only in exceptional cases
Strong communication and time-management skills
Reliable transportation for home visits

Home health care best practices
Patient assessment and progress tracking
Adapting therapy techniques for varied diagnoses
Building trust with patients and families
Accurate documentation and mileage reporting
