Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients recovering from surgery, falls, and other conditions in their homes
Assess each patient's unique needs and create tailored treatment plans that promote independence and mobility
Collaborate with interdisciplinary teams to ensure seamless care coordination across the 60-mile radius
Document progress accurately using HomeCare/HomeBase EMR to support continuity of care
Educate patients and families on exercises and safety strategies to support recovery between visits

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of physical therapy experience with previous home health background preferred
Valid driver's license and auto insurance for travel within the service area
Comfortable working independently with minimal on-site supervision

Knowledge & Skills

Strong clinical skills in physical therapy assessment and treatment
Familiarity with HomeCare/HomeBase EMR documentation
Ability to manage a caseload of 5-7 patients per day
Excellent communication and patient education abilities
Reliable transportation and comfort traveling within a 60-mile radius
