Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's strength, mobility, gait, and rehabilitation potential
Create and adjust individualized treatment plans that help patients regain independence
Educate patients and families on therapeutic exercises and equipment use
Collaborate with physicians and care teams to track progress and adjust care
Supervise home health aides and coordinate with other healthcare professionals
Document clinical notes and progress summaries that support quality patient care

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits
Two supervisory references from your modality (one within 12 months, one within 3 years)

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
Ability to evaluate home environments for safety and rehab potential
Clear written and verbal communication
Understanding of Medicare, Medi-Cal, and Title 22 regulations
