Roles & Responsibilities

• Assess patients thoroughly and create individualized treatment plans to restore function and independence
• Deliver hands-on physical therapy interventions and progress treatments toward measurable goals
• Document evaluations, treatment plans, and daily progress notes in compliance with facility and payer requirements
• Collaborate closely with nursing, rehab team members, and families to coordinate care and transitions
• Meet productivity and quality benchmarks while maintaining patient-centered care
• Follow Continuum Rehabilitation Group policies and procedures to ensure safety and regulatory compliance

Qualifications

• Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) credential
• 1-3 years of experience preferred in skilled nursing or post-acute care settings
• Current state licensure or ability to obtain New Mexico licensure before start
• Up-to-date CPR and any state-required certifications for practice
• Reliable ability to be on-site to receive guaranteed hours and meet productivity expectations

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent professional PT licensure

Knowledge & Skills

• Experience in skilled nursing facility care, including long-term and short-term residents
• Skilled in gait training, balance retraining, transfers, and ADL-focused interventions
• Strong documentation skills and familiarity with therapy productivity expectations
• Good communication and teamwork skills to work with interdisciplinary teams
• Ability to adapt treatment plans based on patient progress and facility workflows
