Roles & Responsibilities

Provide exceptional physical therapy services to residents in a skilled nursing setting
Develop and implement personalized treatment plans to enhance mobility and quality of life
Collaborate with nurses, physicians, and other therapists for integrated patient care
Accurately document patient progress and adjust interventions as needed
Educate patients and families on exercises, safety, and home care strategies

Qualifications

Active Physical Therapist license in Illinois (or willingness to obtain)
New graduates are encouraged to apply
Strong communication and teamwork skills
Compassionate and patient-centered approach

Education

Doctor of Physical Therapy (DPT) degree

Knowledge & Skills

Expertise in geriatric rehabilitation and functional mobility
Proficient in therapeutic exercises, manual therapy, and modalities
Knowledge of SNF documentation and regulatory requirements
Excellent clinical reasoning and problem-solving abilities
