Location: Woodland, California

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with occasional Saturday rotation (once per month)

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess, evaluate, and update therapeutic/rehabilitative/functional status of patients
Develop and implement individualized physical therapy plans of care
Instruct patients and caregivers in use of therapeutic appliances and home exercise programs
Monitor patient progress and report changes to the physician
Supervise Home Health Aides and participate in case conferences
Educate families on goals and rehabilitation techniques
Maintain accurate clinical records and progress summaries

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab setting
Current BLS certification required
Valid Physical Therapy license in California
Excellent oral and written communication skills
Strong organizational abilities

Knowledge & Skills

Home Health and Rehabilitation expertise
Patient assessment and care planning
Physical therapy treatment modalities
Prosthetic device training (as needed)
Understanding of Medicare, Medi-Cal, and OSHA regulations
