Contract Type: Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with occasional Saturday rotations

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in a home health setting
Develop and implement individualized treatment plans to improve mobility, strength, and function
Instruct patients and families on therapeutic exercises and use of equipment
Collaborate with physicians and healthcare team to coordinate comprehensive care
Document patient progress and adjust care plans to achieve optimal outcomes
Educate patients on fall prevention and home safety modifications

Qualifications

Minimum of 2 years of experience as a Physical Therapist in acute care, home health, or rehab setting
Current BLS certification required
Valid California Physical Therapist license
Excellent communication and organizational skills
Ability to travel within Yolo County service area

Knowledge & Skills

Expertise in patient assessment and care planning
Proficiency in physical therapy techniques for home health
Strong documentation and communication skills
Knowledge of Medicare, Medi-Cal, and home health regulations
