Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide home health physical therapy services to a diverse patient population
Conduct start-of-care visits, especially for orthopedic patients when nursing is not ordered
Manage an average of 5 visits per day across Tehama County
Collaborate with intake RNs and office managers to coordinate care
Utilize HCHB (HomeCare HomeBase) for accurate documentation and scheduling
Treat patients with orthopedic conditions, joint replacements, and geriatric comorbidities like CHF and COPD

Qualifications

Active Physical Therapist license in California (or willingness to obtain)
BLS certification from the American Heart Association
At least some home health experience required (less than 2 years considered)
Comfortable driving up to 40 miles per day for patient visits
Reliable transportation and valid driver's license

Knowledge & Skills

Proficiency in HCHB electronic medical records system
Strong clinical skills in orthopedic rehabilitation and geriatric care
Ability to manage multiple comorbidities and create effective care plans
Excellent communication and time management skills
Self-motivated and adaptable in a home setting
