Roles & Responsibilities

• Provide high-quality physical therapy to home health and hospice patients in their homes
• Manage a caseload of 5-7 patients per day, helping them recover from surgeries, falls, and other conditions
• Develop and implement personalized treatment plans to improve mobility and quality of life
• Collaborate with interdisciplinary team members to ensure coordinated care
• Document patient progress accurately using HomeCare/HomeBase EMR system
• Travel within a 60-mile radius around Valley City to deliver care

Qualifications

• Active North Dakota Physical Therapy license required
• Current BLS certification from the American Heart Association
• Valid driver's license and auto insurance (must have upon start)
• Minimum 1 year of experience as a Physical Therapist
• Previous home health experience is a must

Knowledge & Skills

• Expertise in physical therapy techniques for post-surgical and fall recovery patients
• Proficiency with HomeCare/HomeBase or similar home health EMR systems
• Strong communication and patient education skills
• Ability to work independently without direct on-site supervision
• Time management and organizational skills to handle daily caseload effectively
