Roles & Responsibilities

• Conduct comprehensive home health evaluations for orthopedic and geriatric patients
• Develop and implement individualized treatment plans to restore function and mobility
• Provide skilled physical therapy interventions including therapeutic exercises and manual therapy
• Collaborate with interdisciplinary team including RNs, OTs, and SLPs for coordinated care
• Document patient progress in HCHB (HomeCare HomeBase) electronic medical record
• Educate patients and caregivers on home exercise programs and fall prevention

Qualifications

• Active Physical Therapist license in California (or ability to obtain before start)
• BLS certification from the American Heart Association required
• Minimum 1 year of home health experience (travelers with less than 2 years considered with home health experience)
• Experience with HCHB (HomeCare HomeBase) EMR preferred

Knowledge & Skills

• Expertise in treating orthopedic conditions (joint replacements) and geriatric comorbidities (CHF, COPD)
• Proficiency in performing SOC (Start of Care) visits independently
• Strong clinical reasoning and time management skills
• Comfortable driving up to 40 miles per day within Tehama County service area
• Ability to work autonomously with support from intake RN and manager
