Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes throughout Tehama County
Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
Develop personalized treatment plans for patients recovering from joint replacements and managing chronic conditions
Help geriatric patients improve mobility and quality of life while managing multiple health conditions
Document patient progress using HomeCare HomeBase EMR system on provided tablet
Collaborate with intake RN and office manager to coordinate optimal patient care

Qualifications

Current Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Comfortable working independently in home settings
Valid driver's license and reliable transportation

Knowledge & Skills

HomeCare HomeBase EMR system proficiency
Experience with orthopedic and geriatric patient populations
Strong assessment and clinical decision-making abilities
Excellent communication and patient education skills
Ability to manage a caseload of approximately 5 visits per day
