Roles & Responsibilities
- Deliver compassionate physical therapy to patients across four rural counties
- Conduct comprehensive Oasis Assessments and Start of Care evaluations
- Help elderly patients recover from falls and fractures with personalized treatment plans
- Manage a manageable caseload of 4-6 patient visits daily using a fair point system
- Build meaningful connections with patients in their homes throughout Noble, Lagrange, Steuben, and DeKalb counties
Qualifications
- Current Physical Therapist license in Indiana
- Experience with Oasis Assessments and Start of Care documentation
- Comfortable providing care in rural and home settings
- Strong organizational skills for managing daily visits and tablet-based documentation
