Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care, recertifications, and resumption of care visits for home health patients
Perform daily visits averaging 6.5 points per day, including evaluations and treatments
Collaborate with clinical coordinators and sister agencies for support in the field
Complete OASIS assessments and maintain accurate documentation
Provide safety evaluations and occasional hospice teaching visits as needed

Qualifications

Current Physical Therapy license in North Dakota (ND PT License)
BLS certification required
Valid Driver's License and Auto Insurance
Home Health experience and OASIS training preferred
Comfortable with a 50-mile radius rule and no prior affiliation with CommonSpirit/CHI/Dignity in the past 12 months

Knowledge & Skills

Strong clinical skills in home health and hospice settings
Proficiency in OASIS documentation
Ability to manage a caseload of 5-6 visits per day
Excellent communication and teamwork
Self-motivated with ability to work independently in the field
