Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct home health visits including start of care, recertifications, and resumption of care
Perform 6.5 points per day, balancing routine visits with admissions and assessments
Collaborate with a supportive clinical team and coordinate patient care plans
Complete OASIS documentation accurately and timely
Provide safety evaluations and teaching visits for hospice patients as needed

Qualifications

Active North Dakota Physical Therapy license required
Current BLS certification
Valid driver's license and auto insurance
Home health experience strongly preferred
OASIS training is a plus

Knowledge & Skills

Proficiency in home health and OASIS documentation
Ability to manage a diverse caseload including start of care and recerts
Strong communication and organizational skills
Reliable and self-sufficient in a field-based role
