Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM, weekends as needed

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver compassionate home health therapy to patients in their own homes, making a real difference in their daily lives
Develop personalized treatment plans that empower patients to regain independence and mobility
Collaborate with interdisciplinary teams to ensure seamless, holistic care
Educate patients and families on exercises and strategies for long-term success
Monitor progress and adjust therapies to celebrate every milestone

Qualifications

Active Physical Therapy license in good standing
BLS certification
Experience in home health preferred, Epic experience a plus but not required
Completed competency testing, skills checklist, and personal health requirements including vaccinations

Education

Graduate of an accredited Physical Therapy program
Eligible for state licensure as a PT

Knowledge & Skills

Strong clinical assessment and treatment skills
Excellent patient communication and education abilities
Proficiency with electronic health records like Epic
Adaptability to home-based care environments
Commitment to evidence-based practices
